We provide telehealth-based medical care and educational materials.

  • Clinical care is delivered by a licensed and board-certified Physician Associate (PA).

  • Educational content (including blogs, videos, and social media posts) is for informational purposes only and not a substitute for individualized medical advice.

  • You agree not to use our educational content to self-diagnose or delay seeking appropriate medical attention.

2. Eligibility and Use

You must:

  • Be at least 18 years old or have the consent of a parent/legal guardian.

  • Provide accurate, complete information during registration and appointments.

  • Use our services only for lawful, personal health-related purposes.

  • Access our clinical services under false pretenses.

  • All payments are due at the time of service unless otherwise arranged.

  • Missed appointments may result in a fee if not canceled within 24 hours via email at myelmhealth@gmail.com or via phone at 602-556-3434.
